Invitation paragraph We are contacting you to ask if you will take part in a survey exploring the continued long-term impact of coronavirus (COVID-19) on patients with obesity accessing weight management services and bariatric surgery services in the UK. This survey is related to what is happening in your service today compared to the end of the first national lockdown back in July 2020 and is a follow up survey from the initial survey done in the first lockdown. You do not have to have filled out the initial survey in order to complete this follow-up. 2. What is the purpose of the study? This survey aims to explore the continued long-term impact of the COVID-19 outbreak on weight management service and bariatric service including cancellation of outpatient clinics, changes in delivery of service, commissioning and long-term impacts of COVID-19 in the UK. Your information will improve our understanding and help support people with obesity accessing weight management and bariatric surgery services during and after the pandemic. There are no right or wrong answers, we are simply interested in your thoughts. 3. What will happen if I take part? The survey is voluntary, totally anonymous and takes approximately 20 minutes to complete. Throughout the survey, you have the right to withdraw your participation and there are no penalties for withdrawing before completion of the survey. If you withdraw before the end the data will not be used within the analysis. Due to the changing environment related to COVID-19 we are repeating the survey to see how service provision has changed over time. Please use your personal unique identifier you create at the start of the survey to enable you to anonymously repeat follow-up surveys over the course of the COVID-19 outbreak. Keep this safe to allow you to do this. If you completed the survey the first time please put this in so we can track your information. 4. What are the benefits of taking part? Although there is no direct benefit from taking part in the study, completion of the survey is your opportunity to influence the understanding of how weight management and bariatric service have coped and improve future decisions around providing accessibility to weight management and bariatric surgery services. 5. What are the possible disadvantages of taking part? There are no anticipated disadvantages for taking part in this survey. if you experience any problems please contact Dr Adrian Brown. 6. Will my taking part in the study be kept confidential? The survey is hosted by Opinio at University College London which is a secure survey tool. All the data will be stored on a secure password-protected university server at UCL with each participant identified by a unique identifier, all data will be anonymous with no identifiable information being recorded. The data is only accessible by the research team. Anonymised data will be accessed by Scaled Insights, an Artificial Intelligence company that Dr Stuart Flint is the President of, who will analyse open ended questions using their Behavioural Artificial Intelligence softwareData will be held for a maximum period of 10 years after the completion of the research project, after which anonymous electronic data files will be deleted and destroyed. Please also do not provide identifiable information in the open ended questions within the survey. 7. Dissemination of results The finding of the study will be presented at multiple medical conferences and in a peer reviewed publication. 8. What if I have any questions?
