We would like to invite you to participate in this research study about physical activity and exercise for people diagnosed with blood cancer. There is limited information on the opinions of Health Professionals working in haemto-oncology on physical activity and exercise for patients undergoing active treatment for blood cancers. We are interested in gathering information on the views of health professionals working with people with a diagnosis of blood cancer via this brief online questionnaire.

The questionnaire will take approximately 10 minutes to complete. You are free to withdraw at any time without giving a reason.

To be eligible to to take part in this questionnaire you should be:

1. A health professional working in a clinical setting (including medical doctors, nurses, allied health professionals and support/auxiliary health care staff)

2. Currently be in a position where you are working directly with people having treatment or follow up for a diagnosis of blood cancer (leukaemia, myeloma, lymphoma, myelodysplastic syndromes or myeloproliferative neoplasms)

3. Working in the United Kingdom (UK)
